Hold on to your surgeons’ caps because Grey’s Anatomy’s alternate universe episode entitled “If/Then” is slated to air next week and ABC has a “First Look” at the show’s 11-minute opening. And are things ever different in the “what if” of the Grey’s world! However, the burning question is really, “What’s in store for the most adorable girl/girl doc couple Callie and Arizona?”

The teaser opens with Meredith (Ellen Pompeo), in her ever navel-gazing voice over, pondering her life had she made one small decision that would have changed the course of her life.
Cut to a very different Seattle Grace in which Meredith’s mom Ellis (Kate Burton) is still alive and chief of surgery and married to Webber (James Pickens Jr.), Shepherd (Patrick Dempsey) is married to a pregnant Addison (Kate Walsh, who returns for the episode), Christine (Sandra Oh) and Meredith are far from friends, Karev (Justin Chambers) has his act together and has just proposed to Meredith, Bailey (Chandra Wilson) is timid and shy and Lexie (Chyler Leigh) is a drug-addicted mess.
The teaser doesn’t offer too much into the relationship between Callie (Sara Ramirez) and Arizona (Jessica Capshaw) save for a shot of the married ladies spooning in bed in the present before cutting to the alternate reality.
However, in the other universe, Callie is married to Owen (Kevin McKidd) and they’ve got three kids -- a daughter and a set of twins. From the looks of the morning staff meeting Callie and Arizona are professional rivals who continually try to one-up each other. But rivalry can lead to tension of all sorts, so maybe there’s hope for Calzona in the alternate universe.
