Men looking to improve their sex lives by minimizing pain and maximizing aesthetics and pleasure have started getting Botox injected below the belt.
There is Scrotox to make your testicles look smoother and hang lower, and HoleTox, which makes bottoming less painful and more pleasurable. But men can also benefit from getting Botox injected directly into their shafts.
Penile Botox is a new go-to treatment that is gaining popularity among gay and bi men, and not only can it give you a size you've been dreaming of, but it can also help if you struggle to get it up in the bedroom.
But what is penile Botox, and should you book an appointment asap?
What is penile Botox? And why are people getting it?

Penile Botox is a male enhancement treatment where Botox is expertly injected into the penis and works “by relaxing the smooth muscle within the corpus cavernosum, allowing the penis to appear more voluminous in a non-erect state,” explains Aesthetic Nurse Practitioner Chris Bustamante, who is also the owner and CEO of Lushful Aesthetics, which has trademarked the penile Botox treatment ShowTox.
These kinds of Botox injections not only make your penis look bigger but also help to treat "erectile dysfunction by improving muscle relaxation and blood retention,” leading to fuller and harder erections, Bustamante says.
How does penile Botox improve your sex life?

Getting Botox, or ShowTox, to give you a bigger pole can be a huge confidence boost, but it also has the potential to make sex feel even better.
“Increased penile size can enhance sensation for a partner due to added fullness, and many patients report higher satisfaction with their overall sex lives,” Bustamante says. “But this is multifactorial as many aspects can be the psychological improvement and also the extra ‘tightness’ experienced when penetrating a partner due to the size increase.”
But beyond aesthetics and enhanced sensations during sex, penile Botox is also being used for erectile dysfunction for people who haven’t gotten satisfactory results with other ED treatments.
“By helping relax the smooth muscle in penile tissue and improving blood flow, this procedure may support stronger, more reliable erections, making intimacy feel less stressful and more fulfilling,” Bustamante says. “For many, that can translate into increased confidence, reduced performance anxiety, and a greater sense of connection with their partner.”
However, LGBTQ+ sex therapist Ilana Grines warns that if your motivation for getting penile Botox stems from “insecurity, shame or pressure from a partner,” you’re probably better off on a therapist’s couch than a doctor’s exam table.
“So many penis owners are anxious about their size or how their penis will perform,” she says. “The anxiety that accompanies those worries usually shows up in the bedroom and can change the entire sexual experience. Botox can improve the way you feel about yourself, but it will not change the underlying issues present. Many individuals who receive penile Botox struggle with psychological stuff like body shame, desire discrepancy and relationship dynamics, which can’t be solved with an injection. If the underlying issues aren’t addressed, the relief Botox can provide tends to be temporary.”
How does penile Botox impact bottoming?

Penile Botox is all well and good if you’re a top, but bottoms are going to need time to adjust to the larger size, especially if they’re already dealing with tightness, sensitivity, or pain.
“The ass doesn't just automatically adjust because your partner upgraded their erection,” says Dr. Evan Goldstein, the leading anal health expert in the United States who founded Future Method, a sexual wellness brand dedicated to revolutionizing anal sex prep. “It needs time to dilate, maybe even a different lube, and even different positions. I see people skip all of that, and then wonder why something that was supposed to be an enhancement turned into a painful experience. Bottoming is a skill. It requires muscle control, elasticity, and body awareness.”
Which is why he recommends taking a break from bottoming while you train with firmer and larger dilators before you attempt to take your top’s newly enhanced penis. “One partner optimizing their experience without the other being physically or mentally prepared is a recipe for discomfort at best, and injury at worst,” he says.
How much does it cost?

Depending on where you live and the injector you choose, the cost will range from $500 to $3,000. “The cost of ShowTox varies depending on the amount of filler used, but for 300 units, which is the proper dosage necessary to achieve optimal results is $2,500,” Bustamante says. “With the appropriate dosing, penile Botox results will last 3 to 4 months as they do with facial Botox. Patients can always receive more treatments after the results wear off.”
What are the side effects?

According to a study by Oxford University’s Journal of Sexual Medicine into the long term safety and efficacy of using penile Botox for ED, the only side effects participants reported were “mild penile pain on injection and mild penile pain for three days following injection,” with no systemic effects.
After receiving ShowTox, or penile Botox injections, Bustamante warns that “swelling and bruising should be expected for about a week. There may be very mild swelling and bruising that lingers into the second week.”
According to Goldstein, you also have to be on the lookout for issues related to the amount of sex you’re having with your newfound confidence and longer-lasting and harder erections.
“When penile Botox works well, it not only improves one's local blood flow, but also one's overall psyche. That means more fucking for all parties involved,” he says. “That, in and of itself, can lead to potential issues, like prostatitis and penile and anal injuries. So, make sure you take things slow and steady. And since I am an ass man, remember that the ass always deserves the same consideration as everything else in the bedroom. Do the work on both ends — pun fully intended.”
